Why "stubborn" is real — and what helps
Some fat truly resists diet and exercise. The number of fat cells you carry is largely set in childhood and adolescence and stays remarkably constant in adulthood; losing weight shrinks those cells but doesn't remove them, so the body's genetically determined "problem" areas keep their shape even as the scale drops.1 Meanwhile muscle — the scaffolding that gives a body tone and definition — declines by roughly 1% a year from middle age, softening the look long before it affects strength.2,3

Genetics holds the fat, time takes the tone
Two separate things make a midsection or limb look soft: a fat layer that resists dieting, and the muscle beneath it thinning with age. Understanding which one you're fighting — often both — is the whole point of choosing the right treatment.1,2
Your fat-cell count is essentially fixed by early adulthood. When you diet, each fat cell gives up stored lipid and shrinks — but the cells, and their genetically determined distribution, stay put. That's why a flat-everywhere diet still leaves the same stubborn pocket on the lower belly, flanks or outer thighs: you can't spot-reduce away cells that diet only deflates.1
Muscle is the other half. Skeletal muscle starts a slow decline around your 30s — roughly 1% per year from middle age — so the tone and definition that used to show through start to fade, even when your weight hasn't changed much.2,3 This is why "toning" really means two jobs: take down the fat layer that hides muscle, and build the muscle itself.

What the studies actually show
Real numbers from peer-reviewed imaging studies on the treatments above. Read these as separate findings from different studies measuring different things in treated areas — not a head-to-head ranking, and not whole-body weight change.6,8,13
How to read this. These are local changes in the treated area, not pounds lost. They come from different devices, areas and imaging methods (MRI and ultrasound), so percentages aren't comparable across rows. Most are manufacturer-associated studies, and an independent systematic review notes the effect sizes are generally modest with some risk of bias — useful context, not a guarantee.9,10 Individual results vary.

Weight, fat, muscle, skin
A logical sequence for non-surgical fat reduction and toning — do the upstream steps first so the later ones actually show. You don't need everything, and you don't need it all at once.10,12
What's realistic
For a stubborn pocket near your goal weight, expect a visibly slimmer, firmer, more defined contour in the treated area — measured in inches and definition, not pounds. Fat-cell reduction is lasting, but remaining cells can still enlarge with weight gain, and muscle tone fades without upkeep, so results are maintained, not permanent.1,13
Who is not a candidate (or should wait)
These treatments aren't for everyone. RF and HIFEM devices are generally avoided with a cardiac pacemaker, defibrillator or other electronic/metal implant in or near the treatment area; HIFEM over the abdomen is avoided with an abdominal hernia. They're not used in pregnancy or breastfeeding, over active infection or recent surgery in the area, and EmSculpt NEO is cleared up to a BMI of 35 (with arm and flank fat indications limited to lighter skin types and lower BMI). If you have a lot of weight to lose, or significant excess skin, body sculpting isn't the right first step — a medical weight-loss plan or surgical consult is.
