What is Shockwave Therapy?

How does Extracorporeal Shockwave Therapy work?

Extracorporeal shockwave therapy, which is also known as Extracorporeal Pulse Activation Technology or EPAT, is used by a range of professionals such as orthopedists, physiotherapists, sports doctors, aestheticians, and even veterinarians. While the tool has a variety of benefits depending on its use, the way that the technology works is the same.

The primary mechanism used in extracorporeal shockwave therapy is targeted acoustic pressure waves, which are essentially shockwaves, which enhance blood circulation and allow the body to naturally heal quickly. Many consider EPAT a form of regenerative medicine, as it uses these shock waves to help the injured tissue regenerate. This has the largest impact on the musculoskeletal system and is often aimed at injuries and conditions that cause pain to those parts of the body.

As acoustic waves move through the body’s tissue, they work with the body’s naturally high water content to cause reactions to the force. The waves then travel deep into the body’s tissues to gently damage the fat cells and connective tissue that are present. As these tissues heal, the body replaces the damaged tissue with new collagen and tissues, which are more dense than what was there before. This leaves the skin smoother and firmer than it was before treatment, eliminating cellulite.

Many musculoskeletal injuries can be treated in the early stages by simple remedies such as ice and heat, anti-inflammatory medication, and rest. But when these do not work, or an injury progresses into something more chronic, patients find themselves faced with a series of invasive treatment options like surgery. ESWT allows these patients to avoid long healing times and anesthesia while still experiencing benefits such as:

Who Needs EPAT/ESWT?

Anybody who has cellulite they would like to address is potentially a candidate for EPAT treatment, and most people are eligible. However, cellulite therapy using shockwave therapy is not recommended in those who are pregnant, suffering from a clotting disorder, taking anticoagulant medication, actively suffering from an infection or open wound, using a pacemaker, or experiencing cancer at the treatment site. 

When it comes to pain relief, the same restrictions exist, and you should always speak with your medical team about whether ESWT is a good fit for your concerns. Many concerns have been effectively addressed with this therapy, such as rotator cuff injuries, plantar fascia, and other tendinopathy.

What to Expect

Once you have determined that you are a candidate for shockwave therapy and discussed your goals with a provider, you will schedule an initial appointment. Most people will need between eight to twelve treatments, though this can vary widely depending on your exact goals and condition. The entire course of a treatment will take about 10 to 15 minutes. 

To begin the procedure, you will lie down on a comfortable table and expose the area that is being treated. If necessary, you may be asked to change into a gown to allow access to the area. The provider will use a device that looks similar to a traditional ultrasound machine, where they use a handheld Doppler and a gel to apply treatment to the area. It is moved in a circular motion, with the levels being slowly increased to match your tolerance. This is an outpatient procedure and as soon as this is complete, you will be able to return to your day.

Recovery and Results

Because the process of ESWT is non-invasive and causes very little pain, there is no recovery period required after each treatment. Most people will be able to resume their daily activity as soon as they have left the office, though some people will experience mild bruising or sensitivity in the area as it heals. If necessary, you may use over-the-counter medication to reduce this tenderness, but most people do not require even this.

While you may see early results after each session, it is more common to see results after 2 to 3 weeks of treatments. Once you have reached the desired outcome, cellulite reduction can last for several years before additional sessions are needed. 

Those who are using ESWT to treat musculoskeletal conditions and injuries will usually be able to return to physical activity after a day or two of rest to allow the body to begin healing. Depending upon the root cause of the issue, patients may or may not need prolonged therapy in order to manage chronic symptoms as opposed to single courses of treatment.
